	Women 19 through 44
	U.S. Citizen or qualified Immigrant
	Michigan resident
	Have a social security card or have applied for one
	Are not receiving Medicaid
	Meet household income eligibility

Cost

Free to those who qualify and apply.

Covered Services

	Office visits for Family Planning related services
	Physical exam, Pap test
	Birth control method of choice
	Lab tests
	Treatment of some sexually transmitted infections
	Some sterilizations for women 21 years of age and older

Non-Covered Services

	Treatment of infertility
	Abortion

Call your local District Health Department No. 4 office for an appointment

Interesting FACT

Currently, District Health Department No. 4 serves 2,900 women between the ages of 19 through 44. It is estimated that approximately 21,000 women in the District 4 service area will qualify for this program.  District 4 began enrolling women August 1, 2006.
