Covers costs of some basic health care services to include a discount on medications

Eligibility:

	Reside in Alpena, Cheboygan, Montmorency, Presque Isle County;
	Be 19 years of age through 64;
	Have no form of health care insurance;
	Be at or below 200% of poverty (household of two earning $25,660 or less).
	Program limited to 650 individuals this year/Waiting lists are kept on file.

Coverage and Cost:

$5 Copay - Office visits

$5 Copay - Walk-in/Urgent Care visit

$5 Copay - Visit to specialist

$0 Copay - Outpatient lab

$0 Copay - Outpatient X-rays

50% Copay - Covered prescription medications

100% Copay - All other prescription/non-prescription medications

	Inpatient Hospital Services
	Outpatient Hospital Services
	Emergency Room Services
